Job Title: Social Worker - Access and Duty Location: Hackney Pay Rate: £32 per hour (depending on experience) Job Description: Liquid Personnel is recruiting an Adult Social Worker to join its client's Access and Duty team based in Hackney. In this role, you will act as the first point of contact for adult social care services, providing timely and proportionate responses to referrals, crisis situations, and safeguarding concerns. The role involves initial assessments, short-term interventions and signposting to appropriate services, ensuring compliance with statutory duties under the Care Act 2014 and safeguarding frameworks. What will your responsibilities be? Undertake initial assessments of need and risk, including safeguarding enquiries. Provide short-term interventions and crisis management to stabilise situations. Complete Care Act assessments and determine eligibility for services. Develop proportionate support plans and agree outcomes with individuals and carers. Identify and respond promptly to safeguarding concerns, initiating enquiries in line with local procedures. Participate in multi-disciplinary meetings and contribute to joint decision-making. Maintain accurate, timely case records in line with legislation and organisational standards
