Under the direct supervision of the Cumberland Valley District, Jackson County Home Health team leader, administrator and/or their designee, employee will be responsible to: - Provide physical therapy services in accordance with the physician's plan of treatment while maintaining accurate and timely submission of clinical documentation and records according to agency policy (60%) - Maintain communication with physical therapist and other disciplines (10%) - Consult with the physical therapist in the development of patient goals and progress notes (6%) - Keep updated on governing rules and regulations and agency policies and procedures (6%) - Participate with in-service programs and case conferences (5%) - Other duties as assigned by the supervisor or administrative staff (3%) An Equal Opportunity Employer M/F/D
