The Manager, Post-Acute Network Performance is responsible for operationalizing and executing Luminis Health's enterprise post-acute strategy across facility-based and, as phased, home-based partners. This role translates enterprise vision into measurable performance improvement by leading structured governance processes, overseeing partner performance transparency, managing escalation pathways, and ensuring disciplined execution of collaborative engagement models. The Manager is accountable for day-to-day execution of the post-acute network strategy, driving measurable impact in length of stay, readmissions, referral timeliness, and partner reliability under Maryland's Total Cost of Care (TCOC) and AHEAD models. - Execute enterprise post-acute strategy by translating system-level priorities into operational plans, governance processes, and measurable performance initiatives across the partner network. - Operationalize and manage network performance governance, including structured collaborative models such as monthly performance reviews and quarterly partner governance sessions. - Drive Performance Transparency and Accountability: Oversee development, validation, and distribution of standardized partner scorecards and dashboards that monitor throughput, utilization, quality, and outcome trends. - Manage Escalation Frameworks: Implement and enforce defined escalation pathways for recurring operational, clinical, and capacity barriers impacting post-acute transitions. - Advance Throughput Optimization Efforts: Lead initiatives to improve referral timing consistency, acceptance reliability, discharge progression, and reduction of avoidable inpatient days. - Support Level-of-Care Alignment: Monitor and analyze utilization patterns across SNF, IRF, LTACH, and other post-acute settings to identify opportunities for improved clinical alignment and system efficiency. - Facilitate Cross-Continuum Coordination: Collaborate with Transitional Care, Home-Based Care, inpatient leadership, and physician partners to ensure alignment of post-acute performance initiatives with enterprise goals. - Oversee Partner Tiering and Network Evolution: Implement performance-based partner differentiation frameworks and support preferred network development aligned to objective quality and operational criteria. - Lead Continuous Improvement Initiatives: Identify systemic variation and implement structured improvement plans with post-acute partners to address performance gaps and sustain measurable gains. - Support Executive Reporting and Strategic Decision-Making: Prepare and present operational performance insights to Director and executive leadership, translating data trends into actionable recommendations that align with TCOC financial stewardship. - Bachelor's degree required; Master's degree in Healthcare Administration, Nursing, Public Health, or related field preferred. - Minimum of 3 years of experience in healthcare operations, care management, post-acute strategy, or performance improvement leadership. - Experience working with Skilled Nursing Facilities, Inpatient Rehabilitation Facilities, Long-Term Acute Care Hospitals preferred. Demonstrated experience in one or more: Post-acute strategy Population health Care continuum operations Value-based payment models Healthcare network development Experience in Maryland's TCOC or similar global budget environment strongly preferred.
