Core Responsibilities: As a Brain Injury Case Coordinator, you will: - Be responsible for the coordination of supports for individuals seeking services, while providing an understanding of the Brain Injury Waiver application process and eligibility criteria. - Aide individuals with referrals, intake, and eligibility for the Brain Injury Waiver. - Develop preliminary care plans with member/guardians using person centered planning. - Work with Brain Injury providers to coordinate and negotiate required services to implement care plans. Your workload will include: - Helping applicants and assisting them in completing the Brain Injury Waiver Application. - Process, log, and document the status of Brain Injury Waiver Applications. - Participating in teleconferences for members who are placed out of State while documenting their progress in treatment.
