POSITION SUMMARY: The Collaborative Care Behavioral Health Provider provides assessment, diagnosis, individualized treatment planning, and high quality, cost effective psychotherapy and related services to patients in a Collaborative model of care. The position works within their scope of practice, while applying relevant standards of clinical care and professional ethics. The Collaborative Care Behavioral Health Provider provides services alongside primary care providers in a manner that reinforces behavioral health care as part of Gundersen Health System's approach to working with patients to achieve the best possible level of medical, mental and behavioral health. POSITION QUALIFICATIONS: Education and Learning: Required Masters of Science in Social Work or a comparable degree in mental health based on a clinical curriculum that meets State of Wisconsin Licensure and Provider Status Approval requirements. License and Certifications: Required (one of the following) Advanced Practice Social Worker (APSW) Licensed Clinical Social Worker (LCSW) Licensed Marriage and Family Therapist (LMFT) Licensed Professional Counselor (LPC) Marriage and Family Therapist Training License (MFTTL) Professional Counselor Training License (PCTL) Work Experience: Desired 2 years previous work experience with mental health programming and treatment. 1 year previous experience within a hospital, medical group or integrated health care system. Minimum of 3,000 hours of supervised post graduate mental health treatment of populations to be served. PRINCIPAL DUTIES AND RESPONSIBILITIES: Assesses diagnoses, develops individualized treatment plans and provides high-quality psychotherapy and care coordination for patients with mental health problems. This care may include individual, group and family therapies; patient and family education; crisis intervention; consultations; referrals; and follow-up services. Advocates for patients and observes the rights of confidentiality, privacy, dignity and respect while working with patients, employees and other individuals. Utilizes clinical supervisor and collaboration to improve knowledge and skills related to assessment, diagnosis, treatment and care coordination. Meets departmental and organizational standards related to documentation, regulatory compliance and accreditation requirements. Utilization of electronic medical record and other system software as appropriate to provide consistent patient care and communication. Apply customer service principles to interactions with patients, Gundersen Health System departments, external referral sources and other organizations. Contributes to department goals and strategies. Participates in departmental professional, medical, patient or community education activities. Promotes the awareness, recognition and treatment of mental health concerns within other Gundersen Health System departments and the communities served. Performs other job-related duties as assigned. Creates a healthy working relationship with everyone around him or her, as evidenced by zero observations of gossip, behavior demeaning to others, chronic complaining, denigrating ideas or agitating or inciting others. Works to develop trust, respect, support and communication among all members of GBAHC team. Returns equipment to its appropriate location before the end of assigned shift. Demonstrates understanding, compassion, and empathy and communicates this effectively when relating with patient family and co-workers as evidenced by feedback from peer evaluation. Works to enhance teamwork within the department as evidenced by peer evaluation. Attends, at a minimum of 80% staff meetings and educational in-services. If unable to attend in person or via distance ed./conference call, reviews information presented within one week unless previous arrangements made. Stays current on all organization policies by signing 100% of new or revised policies within 30 days of being posted. Follows confidentiality guidelines 100% of the time. Completes facility wide, required safety education.
