We foster a supportive, collaborative environment where your work has a meaningful impact every day. ‎ How will you make an impact & Requirements ‎ Position Overview The Medical Social Worker delivers in-home social services to support clients' emotional, social, and environmental needs. Under the direction of the Director of Nursing, you will conduct assessments, provide counseling, coordinate resources, and contribute to individualized care plans.

Responsibilities

• Conduct psychosocial assessments and evaluate home environments.
• -
• Provide counseling and support to clients and families coping with illness or life changes.
• -
• Assist clients in accessing community, financial, and family resources.
• -
• Participate in developing and implementing interdisciplinary care plans.
• -
• Document visits and clinical notes in the EMR within required timeframes.
• -
• Monitor and report changes in clients' emotional or social status.
• -
• Consult with physicians regarding care plan adjustments.
• -
• Coordinate weekly visit schedules with the Director of Nursing.
• -
• Participate in staff meetings, in-service education, and agency activities.
• -
• Provide monthly supervision to the Social Work Assistant.
• -
• Support discharge planning and continuity of care.
• -
• Maintain professional development and comply with agency policies.
• Job Conditions
• -
• Requires strong communication skills and the ability to perform assessments in varied home environments.
• -
• Occasional bending, lifting (up to 30 lbs.), and assisting with client mobility may be required.
• -
• Must maintain current TB testing.

Requirements

• -
• Master's Degree in Social Work (MSW) from an accredited program.
• Minimum two years of experience, including one year in a healthcare setting.
• Strong interpersonal skills and a client-centered approach.
• Experience with electronic medical records preferred.
