Community Occupational Therapist - Sussex

Responsibilities

• Deliver occupational therapy interventions within an unplanned care service , focusing on admission avoidance and discharge facilitation.
• Complete functional assessments and provide equipment recommendations to promote independence.
• Work collaboratively with the multidisciplinary team to ensure patient-centered care.
• Respond promptly to urgent referrals and manage a varied caseload effectively.

Requirements

• HCPC registration as an Occupational Therapist.
• Experience in acute or community settings , ideally within unplanned care or rapid response services.
• Strong understanding of discharge pathways and admission avoidance strategies.
• Excellent communication and teamwork skills.
• A valid driving licence and access to a car (essential for community visits).
