Organization: Pharmacy Clinical Operations 2 Job Summary: Provides leadership in a specialized healthcare team and the patient to assure optimal cost-effective drug therapy. This includes developing specific pharmacotherapeutic goals, recommending a regimen and developing a monitoring plan to meet the individualized drug therapy needs of the patient. The clinical pharmacist is responsible for leading efforts in the prevention and reporting of medication errors and adverse drug reactions according to policy and works to continuously improve process and practice in the institution to enhance safety of medication therapy. . Position Highlights 7 on/7 off schedule. Hours are 7:00pm - 7:00am. This position will primarily support caring for patients in various ICUs during the overnight hours with some coverage in the emergency department. In addition, this Clinical Pharmacist will have responsibility for consult coverage, clinical questions, and code response for admitted patients in the adult hospital. Opportunities for engagement with other shifts, projects, precepting and committees are available based on individual's interests. We are looking for motivated individuals to help expand clinical scope and practice.
