We deliver 24/7 virtual first and in home medical, behavioral, and social care, working alongside families, providers, and health plans to break down barriers to quality care. The primary location for this role is remote , the expected schedule requirement is 4x10s 8:00 am - 6:00 pm (Tuesday-Friday). Active LA, TN, MO, or TX licensure is required.
Responsibilities
- Manage a panel of complex pediatric patients - Oversee care plans, collaborate with PCPs and specialists, and ensure comprehensive care coordination.
- Work in a multidisciplinary team - Work with pediatricians, nurses, pharmacist, dietitian and social workers to provide wrap-around care in a virtual-first model.
- Empower families - Educate and support parents to navigate their child's health journey with confidence, ensuring long term health success.
- Deliver high-quality virtual care - Conduct virtual enrollment visits, acute escalations, sick visits, and post-hospital follow-ups, and even in-home virtual assessments supported by our Mobile Integrated Care team .
- Ensure continuity of care - Step in when needed to bridge care gaps, coordinating prescriptions, and streamline referrals, to remove barriers to care.
- What You Bring & How You Qualify
- Minimum 5 years' experience with a pediatric population required, Community Based Health or Complex Care strongly preferred.
- You have completed an accredited program for Nurse Practitioner (NP) or Physician Assistant (PA-C) and hold an unrestricted NP or PA state license, including board certification.
- Active TX, LA, MO, or TN licensure required
- Experience and comfort with children with complex medical needs
- Experience with Medicaid and/or Community health preferred
- Virtual care experience a plus.
- Strong desire to improve the quality of life for pediatric patients and their caregivers .
- Ability to lead and guide a clinical team.
- Bilingual Spanish is strongly preferred.
