Social Worker- Mental Health - Kingston Upon Thames

Responsibilities

• Evaluate patients' mental health status, identifying needs, strengths, and areas for intervention.
• Collaborate with the healthcare team to create individualized treatment plans that address patients' mental health needs and goals.
• Respond to mental health crises, providing immediate support and intervention to stabilize patients.
• Attend interdisciplinary team meetings and case conferences to discuss patient care and collaborate with other healthcare professionals.

• Qualifications and Experience :
• To be successful in this role you must have Social Work England registration, be eligible to work in the UK, hold a qualification in Social Work at degree level, and have post-qualified experience.
