Posted Job Title Substance Use Nurse II Job Profile Title Clinical Research Nurse D Job Description Summary The Substance Use Nurse II is an RN who practices as an integral member of an interdisciplinary, collaborative care team focused on providing accessible and site-based primary care services to individuals experiencing homelessness residing in recovery residences. The care team will be focused on expanding low-barrier buprenorphine treatment access and warm handoffs to longitudinal care. On-site clinical services will include RN-led wound care, complex care management, and harm reduction education, among other general primary care needs. The RN will be a champion for trauma-informed care transformation for people with substance use, unsheltered homelessness, and medical complexity. The Substance Use Nurse II colleague demonstrates within the competency domains of Continuous Quality Improvement, Evidence-Based Practice and Research, Leadership, Person and Family Centered Care, Professionalism, Safety, Technology/Informatics, and Teamwork (Definitions listed below). The successful candidate will demonstrate patient-centered, trauma-informed care; a comprehensive understanding of the addiction medicine care continuum, multiple pathways to wellness, and harm reduction strategies; accurate documentation of patient encounters in the data collection system and electronic medical record; comfortability working in community-based settings; and, a deep understanding of the regional addiction care system, and evidence-based care models for opioid use disorder. The RN will be required to travel to community-based locations in Philadelphia. Position is contingent upon continual funding. Job Description Job Responsibilities Provide patient-centered, trauma-informed care, for individuals experiencing homelessness residing in recovery residences. Lead wound care (xylazine wound experience preferred), complex care management, harm reduction education, and general primary care needs. Document patient encounters in data collection system and electronic health record.
