Job Title: Social Worker - Transitions Team Location: Conwy Pay Rate: £43.28 per hour Liquid Personnel is currently recruiting for an Social Worker to join a Transitions Team based in Conwy. What will your responsibilities be? Deliver a high-quality social work service to children and young people who are vulnerable or need support Focus on individuals with physical or mental impairments that significantly impact daily activities Collaborate with citizens, carers, and families to provide support, foster resilience and independence, and ensure person-centred decision-making and outcome planning Complete assessments in line with the Social Services and Wellbeing Act Demonstrate knowledge of the barriers faced by children with disabilities at home and in the community, local authority responsibilities toward disabled individuals and their carers, and relevant legislation and guidance applicable to the role Possess experience working in a multi-disciplinary environment
