About the role

Job Description - Explore and provide psycho-social/emotional care and support to patients and caregivers/family. - Lead in ACP discussions and completion. Complete Preferred Care Plan - Provide Bereavement Support. - Coordinate in-house Financial Assistance for needy patients. - Coordinate Medical Escort & Transport for patients. - Coordinate in referring patients for Respite Care, Nursing Home and other necessities. - Counselling/ Reconciliation - Crisis Intervention - Education, Teaching & Networking Job Requirements - Degree or postgraduate diploma in Social Work - Preferably 2 years relevant experience - Must have interest to work with elderly - Displays compassion, a positive and caring attitude towards patients, internal and external clients. - Excellent communication, interpersonal and problem solving skills - Meticulous, diligent and a good team player. - Adaptable and comfortable with changes *We regret that only shortlisted candidates will be notified.
