In-Home Dialysis Care Partner (1:1 Client) - Thousand Oaks, CA Client: Female adult dialysis patient transitioning home Immediately Location: Thousand Oaks, CA (91359) Compensation : $25/hr Start Date: Urgent About This Role We are looking for a compassionate, dialysis-experienced Care Partner to support a single female patient who is returning home this Friday and will be continuing her dialysis treatments in the comfort of her own home. This is not an agency rotation - you will be paired with one patient, allowing you to build rapport, provide meaningful support, and become a reliable part of her weekly routine. This role is ideal for someone who understands home dialysis equipment, values patient dignity, and wants to make a real difference in someone's life during their transition home.
