Social Worker - Adult Locality

Responsibilities

• Deliver assessments, care planning, and risk management under Self Directed Support and Adult Support and Protection legislation.
• Provide tailored information, advice, and support to service users and carers.
• Develop and coordinate outcome-focused care packages with internal and external providers.
• Maintain accurate records and participate in supervision, training, and team duties.
• Promote independence, choice, and empowerment while identifying and reporting unmet needs.

• Qualification and Experience:
• SSSC registration
• Eligible to work in the UK
• Hold a full UK driving license
• Post-qualified experience
• Degree level or equivalent in Social Work
