Position Summary The Clinical Coordinator, Licensed Practical Nurse will collaborate and coordinate with the Clinical Coordinator, Registered Nurse(s) and Clinical Managers to support the overall clinical operations, functions, and activities of the Home Care team. This position assists with the coordination of clinical operations and documentation processes to support high-quality, compliant home care services. This role utilizes clinical knowledge and critical thinking skills to manage patient supply ordering and assist with quality assurance processes within the EMR, coordinate Start of Care (SOC) activities, and collaborate with interdisciplinary teams to ensure timely and effective patient care. The Clinical Coordinator, Licensed Practical Nurse also supports agency performance improvement initiatives through QAPI audits, patient care coordination, and ongoing communication with physicians, staff, patients, and community resources. Education Graduate of an accredited school of practical nursing. Current State of New Jersey license (or legal permission to work from the New Jersey Board of Nursing) required. Current and active Basic Life Support (BLS) certification required.
