  • Skill in collecting, reviewing and analyzing client case records in order to make recommendations on ways to simplify reporting and documentation practices and procedures.

This position provides support, education, and information to families to promote self-advocacy and access to available resources and services. The Lead Extended Services Coordinator will develop and prepare reports, correspondence, and evaluations regarding client status, problem resolution, and the implementation of long-term plans and strategic initiatives. The successful candidate will collaborate with families, community partners, and service providers to ensure effective coordination of services and positive outcomes for clients. Essential Functions Provides home and natural environment, center-based family centered services to eligible Part C infants & toddlers and their families. Develop collaborative relationships with families while maintaining professional boundaries. Determine with families their concern, priorities and needs of the child and family. Jointly develop and revise Individualized Family Service plans.
