Mobile Mental Health Clinician, Lorain

About the role

Stepping Stone Behavioral Health is seeking dedicated and compassionate Licensed Social Workers (LSW/LISW), Licensed Professional Counselors (LPC/LPCC), or Licensed Chemical Dependency Counselors (LCDC II, LCDC III, LICDC) to join our outpatient behavioral health team . This mobile clinician role provides mental health and substance use counseling directly inside skilled nursing and long-term care facilities , offering meaningful support to residents coping with emotional, behavioral, and substance-related challenges. At Stepping Stone , we specialize in providing behavioral health and substance use treatment within skilled nursing facilities , supporting individuals with complex medical and emotional needs. Responsibilities

• Conduct bio-psychosocial assessments and develop individualized treatment plans.
• Provide individual and group counseling , teaching coping and recovery skills.
• Identify resident needs, set measurable goals, and guide them toward emotional stability.
• Participate in treatment team meetings and collaborate with facility staff and community partners.
• Maintain timely and accurate clinical documentation in EMR systems.
• Assist with telehealth sessions and submit required daily reports.
• Facilitate safe discharge planning and coordinate aftercare services.
• Uphold confidentiality, professionalism, and compliance with facility and state regulations.

Requirements

• Must be 21 years or older with a valid driver's license .
• Active Ohio license : LSW, LISW, LPC, LPCC, LCDC II, LCDC III, or LICDC.
• Ohio Medicaid Billing Number and NPI Number required (or ability to obtain).
• Minimum of an Associate's Degree (Bachelor's or Master's preferred).
• Strong interpersonal, organizational, and documentation skills.
