About the role

The RN Patient Flow Coordinator is responsible for optimizing patient throughput and ensuring efficient patient movement throughout the facility. Responsibilities

  • Patient Flow Management
  • Coordinate and prioritize patient admissions, transfers, and discharges across all units.
  • Monitor real-time bed availability, staffing levels, and unit capacity to facilitate optimal patient placement.
  • Serve as the central communication hub for bed management and patient movement.
  • Collaborate with Emergency Department, perioperative services, and inpatient units to reduce wait times and bottlenecks.
  • Clinical Coordination
  • Review patient acuity and ensure appropriate level-of-care placement.
  • Utilize clinical judgment to assign patients based on medical needs, safety considerations, and organizational protocols.
  • Support continuity of care by ensuring timely transitions between departments.
  • Collaboration & Communication
  • Communicate effectively with physicians, nursing leadership, case management, and support services.
  • Facilitate interdisciplinary discussions to resolve patient throughput challenges.
  • Escalate capacity concerns and coordinate surge planning as needed.
  • Operational Efficiency
  • Track and analyze patient flow metrics to identify trends and opportunities for improvement.
  • Assist in implementing strategies to improve throughput, reduce length of stay, and enhance patient experience.
  • Ensure compliance with hospital policies, regulatory requirements, and patient placement guidelines.

Requirements

  • Education: Associates Degree Nursing required.
  • Nursing Licensure: Current State of New Mexico or Compact State Nursing License required
  • Certifications: BLS required
  • Specialty Certifications in area of practice and/or management preferred.
  • Experience: 2 years experience as a clinical nurse.
  • Skills: Requires detailed knowledge of patient flow through the hospital in terms of what and how work is to be done as well as why it is done, this level includes interpretation of data.
