Responsibilities
- Undertake comprehensive psychosocial assessments and formulate appropriate intervention and discharge plans.
- Apply advanced clinical reasoning to support patients with complex psychosocial needs.
- Liaise effectively with patients, families, carers and multidisciplinary team members to coordinate care and facilitate positive outcomes.
- Support safe discharge planning and continuity of care.
- Participate in quality improvement, service development and research activities.
- Provide supervision and contribute to student and clinical education programs as required.
- Foster strong working relationships within the department, across the organisation and with community-based services and stakeholders.
- Your Contribution
- To be successful in this role, you will have:
- A Bachelor Degree or Qualifying Master's Degree in Social Work.
- Significant social work experience within a hospital or related inpatient healthcare setting.
- Demonstrated ability to undertake complex psychosocial assessments and interventions.
- Strong communication, interpersonal and relationship-building skills.
- Experience working collaboratively within multidisciplinary teams.
- Current National Criminal History Check, or willingness to obtain.
- Experience in sub-acute care and/or Geriatric Evaluation and Management (GEM) services will be highly regarded.
