Liquid Personnel is recruiting Social Workers to join its client's Adult Hospital Discharge team based in Manchester. What will your responsibilities be? Carry out Care Act assessments, strengths-based and person-centred, for citizens recently discharged from hospital. Complete timely reviews of hospital-arranged care packages, assessing for long-term support before transfer to community teams. Conduct Mental Capacity assessments, Best Interest decisions, and safeguarding work where required. Work independently in a hybrid model, seeking guidance for complex cases, with no duty function
