Unit Coordinator, Patient Care Coordinator - Plastics Clinic (GEN)

Responsibilities

• Act as an expert clinical resource in collaboration with CPL and Manager to the clinical members for the Plastics department and Surgical Clinics

Requirements

• Current registration with College of Nurses of Ontario as a Registered Nurse
• Recent experience (within last 3 years) within outpatient ambulatory clinic and surgical procedure experience
• BCLS certification- ACLS certification an added bonus
• Demonstrated commitment to professional practice standards and patient care needs
• Demonstrated expertise in management of wound care and surgical clinic patients
• Excellent physical assessment skills, critical thinking and professional practice judgment expertise
• Strong content knowledge surrounding the clinical needs of the patient population, Strong understanding of teaching/learning theories, tools and techniques
• Exceptional understanding and demonstrated experience in coaching and mentoring of clinical staff
• Strong ability to read/analyze and utilize research/evidence, and experience utilizing quality management data for monitoring and identifying trends and impacts with the ability to make suggestions to move forward with action plans
• Effective interpersonal, communication, negotiations, conflict management/resolution and presentation skills
• Demonstrates values consistent with SHN's Code of Conduct and is committed to fostering a healthy and positive work environment in accordance with Health & Safety policy and legislation.
• Utilize protective equipment provided and identify and reports hazards
• Engages in ethical conduct and strives to understand and appreciate the diversity of our patient/staff population and community

• Undergraduate Degree in Nursing or Masters prepared
• Member of RNAO, wound care certification
• Ability to speak another language

The Patient Care Coordinator will liaise with CPS, inpatient teams, surgeons and Surgical Clinics. The goal of this role is to facilitate and promote clinical effectiveness for efficient patient care.
