Job Description The Oncology Pharmacist will provide oncology pharmaceutical care services using approved protocols in collaboration with other health professionals, including supervision of the preparation of commercial IV admixtures and continuous quality improvement. Independent thinking and decision-making is required. Performs these duties in the Infusion Pharmacy assigned. Essential Job functions : Process and review all treatment orders accurately and efficiently. Verify appropriateness during order entry. Assure that all medications meet the highest standards in quality and preparation of final dosage form, neatness and labeling. Understands the functions of each institutional and pharmacy information system and demonstrates an ability to effectively use each system. Anticipates and corrects problems and performs appropriate follow up. Supervises production of all types of admixtures, verifies accuracy of procedures, ensures accurate labeling and doses prior to prepared doses leaving the area. Manages time efficiently to maintain high productivity through oversight of pharmacy technician(s). Establishes and maintains effective communication with pharmacy staff members, other health care disciplines and patients providing drug information. Participate in medication safety initiatives in collaboration with the Manager of Oncology & Infusion Pharmacy Services. Teaches and instructs pharmacy and ancillary staff on proper chemotherapy regimen interpretation, order validation, drug preparation and monitoring parameters Reviews patient medication regimens for clinical effectiveness, drug selection, dosing, contraindications, side effects, potential drug interactions as required. Conducts patient medication history inquiries and drug information requests upon referral from physicians and other medical personnel. Analyzes and employs a systematic, efficient, and thorough procedure for retrieving drug information. Formulates response and follows up consistently on drug information recommendations and assesses effectiveness. Collaborate with other health care professionals to plan, monitor, review, or evaluate the quality or effectiveness of drugs or drug regimens, providing advice on drug applications or characteristics. Communicates and documents findings with prescribers and provides appropriate alternatives to current treatment plans as needed in the EHR. Identifies, resolves, and prevents potential and actual medication-related problems including: untreated indications, improper drug selection, sub-therapeutic dosage, overdose, failure to receive medication, adverse drug reactions, drug interactions, and medication without indication. Maintains all processes & environment in compliance to USP-797 & USP-800 Guidelines. General Job functions: Develops and participates in operational and clinical projects as determined by current departmental needs and ongoing projects. Assures ongoing performance improvement of the pharmaceutical care services provided. Other job duties as required. OR State Board of Pharmacy licensed required. Ability to communicate in English, both orally and in writing required.
