Responsibilities

  • Develops organization-wide reports, tools, and dashboards across various platforms (i.e., Vizient, Epic, RLDatix) in collaboration with stakeholders
  • Creates high level executive summaries of system performance related to public reporting (CMS Stars, Leapfrog, US News, etc.) with recommended path toward improvement based on findings
  • Participates in and contributes to value improvement activities through synthesis of quality, financial, supply, and other data to determine opportunities and actionable items.
  • Serves as liaison between stakeholders (e.g., clinical leaders, physicians, payers and Health Information Technology), to ensure accurate data elements for reporting are identified to maximize reporting effectiveness and utilization.
  • Identifies supportive reports required and facilitates creation to advance internal scorecard performance and achievement of goals across multiple areas.
  • Analyzes data pertinent to identify opportunities for improvement based on internal goals, external/internal benchmarking, industry standards and payor/external regulatory rating systems
  • Manages performance and process improvement projects, including those at the hospital, region, and system levels
  • Identifies, communicates and engages with key stakeholders required to address issues; moves projects forward according to defined goals and timelines. Identified action steps required when goals not achieved.

Requirements

  • Education:
  • REQUIRED: Bachelor's Degree in clinically-focused area such as RN, OT, PT, pharmacist, health information technology, or other healthcare-related degree.
  • PREFERRED: Master's Degree in clinically-focused area such as RN, OT, PT, pharmacist, health information technology, or other healthcare-related degree.
  • Certification:
  • Minimum Certification Requirements
  • CPPS/CPHQ, LEAN/Six Sigma Certification and/or similar training highly preferred.
  • Minimum Work Experience
  • 5 years of healthcare experience including data analytics or use of data to drive performance improvement
  • Strong analytical skills to analyze complex datasets and identify trends/solutions to data quality issues
  • Must be fluent in process improvement methods and tools to evaluate, measure capability, and redesign operational and clinical processes.
  • Candidate will be able to interpret and display data in manner that identifies areas of opportunity and engages leaders to identify tactics for care improvement.
  • Skilled in Utilizing Process/Performance improvement methodologies (IHI, Lean, Six Sigma, etc.)
  • Knowledge of data governance policies and procedures
  • Analytic, problem solving, and issue resolution skills
  • Demonstrated written and verbal communication skills with ability to communicate data issues and solutions to both technical and non-technical staff

Hourly Pay Range: $42.54 - $65.94 - The hourly pay rate offered is determined by a candidate's expertise and years of experience, among other factors. Position Highlights: Position: Program Manager, Quality & Care Transformation Location: Evanston, IL Full Time: 40 hours/week Hours: Monday-Friday, 8:00a-4:30p A Brief Overview: The Program Manager, Quality & Care Transformation is responsible for leading clinical quality innovation and performance improvement strategy and care transformation initiatives. This role supports the team to develop and implement organization-wide performance improvement, and clinical innovation projects. Collaborates on development of metrics, data, tools and processes to develop, implement actions and monitor performance. Coordinates with clinical areas to assure ongoing accreditation compliance. Develops strong collaborative relationships with operational leaders, physicians across the organization and within the Clinically Integrated Network (CIN). Works closely with physicians, office staff, Clinical Integration Network (CIN) medical leadership, payors and administrative data vendors to assist in facilitating meeting established quality improvement study submissions and targets. This role requires experience and expertise that aligns with the autonomy and independence necessary for specific work assignments and decision-making, which is crucial for the speed and success of projects and outcomes.
