SIMBIE AI - VP of Engineering NYC - Full-Time - Founding Team About There are never enough staff in a medical practice, physician or non-clinical, to handle all the work to be done - from reviewing patients' test results, to calling patients to discuss them, to answering patient calls about scheduling and clinical questions, to calling patients to check if they're taking meds rights. Simbie is the clinically intelligent AI voice agent that automates and expands all of this work, saving practices time and money while adding patient touchpoints that save lives, but that practices can't afford human staff for. We're the first platform executing AI workflows requiring real clinical judgment, directly in the EHR. We're YC-backed, distribution is proven, and we're signing customers faster than we can currently bring them live. That's the problem we need you to solve. The Role You'll own the full technical vision and be genuinely hands-on - architecture decisions in the morning, code review in the afternoon. You'll partner directly with a physician and a clinical operator who've already proven distribution. Not a figurehead: you build, you ship, you talk to customers. Builder first. You sketch the MVP while the physician is still describing the workflow. Fast, demo-driven, unafraid. Customer-close. You feel physicians' pain directly and translate it into product. What You'll Own Engineering team - hire and lead from scratch, set the culture Data strategy - build the first between-visit clinical data layer; use it to improve models over time EHR integrations - repeatable framework across EHRs; strategic calls on prioritization and TAM Technical roadmap - architecture, AI/ML strategy, infrastructure What We're Looking For 7+ years engineering, 3+ in technical leadership; startup experience essential Deep AI/ML: LLMs, RAG, fine-tuning, agents, vector databases Python primary; React / TypeScript a plus EHR integrations (FHIR, HL7) and healthcare data - strongly preferred Shipped production AI systems; comfortable with cloud infra and MLOps Bonus: voice AI, multi-modal models, prior founder/founding engineer experience What We Offer Founding team equity + competitive salary Full ownership of technical decisions Hands-on build of core product to scale Ownership over building out and leading the technical team Small, high-trust team; latest AI tools and compute power Join Us We've done the hardest thing in healthcare: proven distribution.
