This role focuses on ward-based medicines management activities, supporting safe and effective patient care through accurate medicines reconciliation, patient counselling, and clinical support. The successful candidate will play a key role in ensuring medicines are used safely and efficiently across inpatient wards, working closely with pharmacists and multidisciplinary teams.
