Requirements

• Degree in Social Work (or major in Social Work) from a recognised institution
• 2-3 years of relevant experience in social work, preferably in healthcare or community settings
• Experience in case management,including psychosocial assessment, intervention, and care planning

Additional Information

Conduct comprehensive biopsychosocial assessments for patients, residents, clients, and caregivers, and implement appropriate interventions in line with established social work professional standards. Manage the full spectrum of social work services, including assessment, care coordination, intervention planning, counselling, emotional support, monitoring, and outcome evaluation. Address clients' psychosocial and financial concerns, while contributing to discharge and transition planning as part of integrated care management. Work collaboratively with healthcare professionals such as physicians, nurses, therapists, and other allied health staff to deliver coordinated and person-centred care. Engage and reach out to individuals and communities who may benefit from group work interventions and support programmes. Provide mentorship, training, and shared supervision for social work students on field placements and newly appointed Medical Social Workers, fostering professional growth and adherence to best practices.
