Liquid Personnel is currently recruiting for an experienced Qualified Social Worker to join the South Community Team within Adult Social Care on a 3‑month contract with South Gloucestershire. This is a key frontline position within a locality Adult Social Care team. You will undertake social work assessments, provide advice and guidance, and support individuals to achieve positive outcomes. You will work as part of a multi-disciplinary team, collaborating closely with colleagues across health, social care, and the voluntary sector to ensure effective communication and the delivery of best practice. What will your responsibilities be? Undertake assessments of need and develop person-centred care plans Review and monitor care plans, responding proactively to changes in needs Support and evaluate reablement programmes to promote independence Manage your own caseload, ensuring timely and effective interventions Carry out risk assessments and safeguarding work, including self-neglect cases Work in partnership with external agencies to coordinate support and services Promote awareness of care issues and ensure safe, effective practice
