Position Overview As an Associate Care Manager, you play a key role in supporting patients through their care journey, helping ensure they feel informed, supported, and connected at every step. Working alongside an interdisciplinary team, you assist in coordinating services, addressing barriers to care, and contributing to safe, timely transitions between care settings. Primary Department, Division, or Unit: Care Management, UHS Binghamton General Hospital Primary Work Shift: Day Rotational Regular Scheduled Weekly Hours: 40 Compensation Range: $27.80 - $41.69 per hour, depending on experience ----- Job Responsibilities: Coordinates timely, quality, cost effective hospitalization for patient through identification of discharge planning needs, referral and follow-up of identified patients. Collaborates with the Nurse Care Manager to develop and implement a comprehensive plan of care through the interdisciplinary team process in conjunction with the patient and family. Assists in data collection as directed assuring integrity and timely completion. Provides education to patients and families regarding post-acute care needs. Serves, as patient advocate by assuring care rendered is appropriate, timely, and confidential. Identifies and fosters acceptance of appropriate levels of care to meet the anticipated and/or established continuing care plan. Participates in community organizations and activities serving the patient population. Keeps abreast of research, literature and developments in the Continuing Care field. Attend appropriate continuing education programs. May assume additional responsibilities, as requested. Position Qualifications: Minimum Required: Associate in applied science degree in related field or five (5) years experience in acute/tertiary facility and/or community health nursing.
