Responsibilities

  • Weekly patient rounding-coordination of care and communicating plan of care to the patient/family and multidisciplinary team.
  • Navigate the continuum of care for patients including the coordination of post-op/procedure visits, Acute Rehab, Home Care and/or Skilled Nursing Facility (SNF).
  • Optimize post hospital care coordination including Tele-monitoring and specialist referrals.

  • The Stroke Program Coordinator/CNS is accountable for the accreditation and certification of the program, including all supporting data, process improvement, and multidisciplinary evidence-based practice development, including:
  • Coordination with Medical Staff Stroke Program Director for maintenance and growth of the program; for evaluation of outcomes, and for improvement of outcomes
  • Community screening events & lectures
  • Consult with healthcare team and patient during hospitalization
  • Post-hospitalization follow-up
  • Develop or collaborate on patient teaching materials
  • Secondary responsibilities include staff development regarding care of the Stroke patient and development of related materials and input on related PI projects.
  • The Behavioral Health CNS coordinates activities for the Behavioral Health Services and the Medical Center:
  • Facilitating admissions between Behavioral Health Services and the Medical Center
  • Consultation to the Medical Center regarding psychiatric issues and substance abuse
  • Ongoing staff education and support
  • Quality management as it relates to the position

Requirements

  • Master's degree in Nursing.
  • Must have at least 24 months clinical nursing experience in area of specialty.
  • Demonstrated ability to produce results and coordinate projects
  • Experience of expertise in quality improvement and evidenced based practice.
  • Prior CNS, NP or nursing education experience preferred.
  • Excellent verbal and written communication skills, problem solving, conflict resolution skills.
  • Computer skills that include word processing and database management
  • License/Certification/Registration Requirements
  • Current California RN license.
  • California certification as a Clinical Nurse Specialist or California licensure as a NP Current
  • BLS certification
  • Department Specific:
  • Stroke Coordinator/Neurovascular
  • Three (3) years of recent experience in acute nursing and demonstrated clinical com

The CNS/NP plans, implements, coordinates and evaluates nursing care and clinical programs for patients. In addition, the CNS/NP advances nursing practice and optimizes patient outcomes through education, leadership and participation in clinical research in collaboration with Service Line Leader and Physicians. The CNS/NP practices in accordance with the California Nurse Practice Act, ANA Code of Ethics, and the ANA Bill of Rights for Nursing as well as the Hospital/Department policies. The CNS/NP provides care in a collaborative manner with other health team members to ensure optimal, individualized patient care and outcomes.
