Villa Colombo Toronto- Nurse Practitioner- Permanent Part-Time

Responsibilities

• Provide clinical leadership support for the home's interdisciplinary team to provide proactive screening and assessment, timely episodic care and specialist referrals follow-up care, ongoing chronic disease management, palliative and end-of-life care.
• Provide leadership and mentoring support to staff to enhance their knowledge, assessment skills and ability to care for residents, optimizing resident health care outcomes - Provide care for a select group of residents in consultation with the physicians and the interdisciplinary team
• Participate in the development and delivery of clinical skills education for staff, residents and families.
• Participate in creating an organizational environment that supports safety and quality of resident care and life, collaborative practice, and professional growth.
• Maintain current knowledge of care-related practice, issues and activities at Villa Colombo
• Participate in employee orientation/training to ensure ongoing development of clinical skills and a commitment to the philosophy, values, policies and clinical practices.
• Participate in identifying, analyzing and interpreting trends in resident care and professional nursing practice to determine priorities for educational programming for team members, residents and families. - Develop linkages with external partners, as appropriate, to ensure continuity of care
• Attend and/or support internal and external committees as required.

Requirements

• Minimum one year clinical and/or working experience in geriatric, rehabilitation, or long-term care nursing.
• Leadership knowledge and/or experience are considered an asset.
• Certifications obtained or in progress that includes gerontology, mental health, behavioral support and palliative care is an asset.
• Current registration with the College of Nurses of Ontario as a Registered Nurse in the Extended Class (Primary Health Care or Adult) and entitled to practice.
• Working Knowledge of the; Long Term Care Act, Program Standards for Long Term Care, Health Disciplines Act, Occupational Health & Safety Act, Pharmacy Act, Food and Drugs Act (Canada), Narcotic Control Act (Canada) and Coroners Act, is considered an asset Must be available to be on site.
• Ability to speak Italian language is an asset.
