In this role, you will: Complete strengths‑based Care Act assessments and develop person‑centred support plans. Manage a varied caseload, including safeguarding, risk assessment, and complex decision‑making. Work collaboratively with health partners, providers, and families to promote independence and ensure safe, effective support. Qualifications & Experience: To be successful in this role you must have: Social Work England registration Eligible to work in the UK Hold a full UK driving licence Post-qualified experience Degree level or equivalent in Social Work
