Responsibilities

• Support Medical Social Workers in patient care coordination and case management
• Manage routine inpatient and outpatient cases independently
• Conduct financial assessments for assistance schemes
• Facilitate discharge planning and coordinate community support services
• Conduct field visits and follow-up for discharged patients
• Liaise with community partners and social service organisations
• Support administrative and operational duties within the department
• Provide guidance and support for new team members

Requirements

• Diploma in Social Work, Health Sciences, Nursing, or related discipline
• Minimum 1 year of experience in healthcare or social service settings
