We are expanding with a new, one-million-square-foot, 15-floor, state-of-the-art facility on Cleveland Clinic Main Campus designed to elevate care for complex neurological conditions - including spine injuries, epileptic seizures, movement disorders, multiple sclerosis and stroke. This advanced building brings every stage of care under one roof, featuring dedicated spaces for outpatient services, surgery, stepdown, ICU and epilepsy monitoring, as well as additional operating rooms and expanded ambulatory, infusion, imaging, EMG, sleep and rehabilitation services. We are welcoming new caregivers to collaborate within our multidisciplinary teams and contribute to nationally recognized, innovative neurological care. As an RN Case Manager, you will collaborate with interdisciplinary healthcare teams across the continuum of care to coordinate seamless transitions and deliver high-quality, patient-centered care for adult neuro patients. In this role, you will apply your clinical expertise, critical thinking, and compassionate approach to advocate for patients and families while conducting holistic assessments, developing individualized care plans, and empowering patients to successfully self-manage their healthcare needs. Your work will play a vital role in promoting continuity of care, improving patient outcomes, and ensuring safe and effective transitions across healthcare settings. This neuro-specific position will be embedded directly within the nursing units of the neuro building, providing close collaboration with physicians, nursing staff, therapists, and other care team members. Candidates with prior neuro experience are strongly preferred, though previous case management experience is not required if supported by a strong neurological clinical background. Team members will complete a comprehensive 8-12 week onboarding and training program alongside experienced hospital case managers, including dedicated neuro-focused education and hands-on training designed to support long-term success within this specialized patient population. A caregiver in this position works days| full-time| Mon-Fri| 8:00am-4:30pm w/three weekend shifts per quarter. A caregiver who excels in this role will: Ensure compliance with standards of the accrediting bodies, state mandated revised code and the CMS Conditions of Participation. Promote interprofessional synchronization throughout the acute episode. Complete effective handoffs to the next level of care or community to ensure continuity of care. Proactively assess, identify and work to mitigate the risk posed by barriers to patients' transitions of care. Work with the patient and/or care partner(s) to develop a comprehensive, individualized transitional care plan. Participate and lead in patient rounding activities, huddles and committee meetings. Minimum qualifications for the ideal future caregiver include: Graduate from an accredited school of Professional Nursing (Diploma, ADN, BSN program) Current state licensure as a Registered Nurse (RN) Basic Life Support (BLS) Certification through the American Heart Association (AHA) or American Red Cross Three years of recent RN experience Preferred qualifications for the ideal future caregiver include: Bachelor's of Science in Nursing (BSN) CTM, ACM, CCM or AHA Certification by an approved entity Two years of recent experience in utilization review, quality or care management Knowledge of acute care and accreditation requirements Experience in discharge planning and quality improvement Physical Requirements: Requires normal or corrected hearing and vision to normal range. Ability to perform work in a stationary position for extended periods. Ability to communicate and exchange accurate information Ability to work with physical records, such as retrieving and filing. Ability to operate a computer and other office equipment. Ability to travel throughout the hospital system. Requires normal or corrected hearing and vision to normal range.
