About the role

Responsibilites: Support the clinical team in advising clients on financial assistance schemes, ensuring timely updates for means testing reviews and subsidy renewals Assist the clinical team with administrative processes to facilitate effective service delivery to clients and caregivers Manage the onboarding of new clients, including scheduling and rescheduling of initial assessments, processing subsidy applications and responding to rehabilitation-related enquiries Provide support in the initial assessment of client and caregiver needs, including basic case management under the guidance of a Case Manager or Social Worker Enhance clients' and caregivers' ability to cope by coordinating and streamlining access to relevant community services Provide clients and caregivers with appropriate information, guidance, and referral support Conduct joint home visits with Case Managers, Social Workers or members of the multidisciplinary team when required Support the planning and execution of events or projects related to client and caregiver support Participate in case discussions with the clinical team Prepare, consolidate and monitor monthly reports and relevant statistics Ensure timely and accurate documentation in both digital systems and physical records Perform any other duties as assigned by the reporting officer Requirement: Diploma or Advanced Diploma, preferably in a healthcare-related discipline Minimum 1-2 years of relevant experience in the healthcare or social services sector Compassionate and patient individual with a gentle disposition, strong empathy and active listening skills Team player with strong interpersonal and communication abilities Familiar with or able to quickly understand the healthcare and social services landscape Demonstrates a positive attitude, adaptability and maturity in a dynamic work environment Interested candidates who wish to apply for the advertised position, please click on "Apply Now" or WhatsApp us at 8909 8083 for quicker respond.
