Founded in 1793, BCHD is the oldest health department in the country, with over 800 employees and an annual budget of $208 million, that aims to protect health, eliminate disparities, and ensure the well-being of every resident of Baltimore through education, advocacy, and direct service delivery. BCHD's wide-ranging responsibilities include maternal and child health, youth wellness, school health, senior services, animal control, restaurant inspections, violence prevention, emergency preparedness, STI/HIV treatment, and acute and chronic disease prevention. The Extended Services Coordinator will advise clients on available City, State and private services and programs; provides clients with handouts, brochures and related documents for their information and use. This position is responsible for maintain accurate, up-to-date and detailed client case records and files and ensure the confidentiality of client case records and files. Essential Functions Develop collaborative relationships with families while maintaining professional boundaries. Determine with families their concern, priorities and needs of the child and family. Jointly develop and revise Individualized Family Service (IFSP) plans. Effectively monitor and adhere to the required timelines for Part C and Part B and maintain all required paperwork and documentation in the Maryland Online IFSP.
