Requirements

• Proven experience within hospital discharge settings
• Strong working knowledge of Care Act, MCA, safeguarding, and discharge processes
• Ability to manage a fast-paced caseload and work under pressure
• Excellent assessment and decision-making skills
• Available to start immediately
• Qualifications & Experience:
• To be successful in this role you must have,
• Social Work England registration
• Eligible to work in the UK
• Hold a full UK driving licence
• Post-qualified experience
• Degree level or equivalent in Social Work

Liquid Personnel is seeking an experienced Social Worker to join its clients Hospital Discharge Team, supporting citizens across discharge pathways. What will your responsibilities be? Assess and support citizens on discharge from hospital across three pathways Complete post-discharge assessments and reviews Undertake Care Act Assessments (CAA) and support planning Carry out Best Interest (BI) decisions and Mental Capacity Act (MCA) work Lead on safeguarding enquiries and interventions Provide duty cover as required Attend Quality Assurance panels for funding provisions Support Discharge to Assess (D2A) pathways
