Preparing for Adulthood (PFA) Social Worker Requires experience working with young adults in transition or leaving care Must have conducted focused reviews and managed direct payments Strong knowledge of Care Act assessments, MCA, and Best Interests decisions essential Familiarity with CHC checklists and 3 days per week in-borough (office/visits) mandatory
