Responsibilities
- Working with families and providers to develop functional Individualized Family Service Plans (IFSPs)
- Work with multidisciplinary teams to determine families' needs for support and services
- Conduct home visits
- Coordinate and facilitate services and transition planning
- Participate in the authorization and monitoring of early intervention services
