Job Order Hourly Rate of Pay: $24.41 (Based on education and/or years of relevant work experience reflected on the application). Responsibilities

• Working with families and providers to develop functional Individualized Family Service Plans (IFSPs)
• Work with multidisciplinary teams to determine families' needs for support and services
• Conduct home visits
• Coordinate and facilitate services and transition planning
• Participate in the authorization and monitoring of early intervention services
