As an RN Case Manager, you will collaborate with interprofessional teams across the continuum of care to facilitate effective transitional care coordination. In this role, you will develop and manage individualized discharge plans and coordinate patient transitions to home (with or without services), skilled nursing facilities (SNF), rehabilitation centers, or long-term care settings. You will ensure all post-acute services are arranged, including home health, durable medical equipment (DME), and transportation. Responsibilities also include educating patients and families on discharge plans, available resources, and post-acute care options, while setting clear expectations around timelines and care transitions. You will address concerns or resistance related to discharge planning while promoting patient advocacy, holistic assessment, care planning, and empowering patients and families to self-manage their care. A caregiver in this position works day shift from 8:00 a.m. -- 4:30 p.m. with one weekend shift every three weeks. A caregiver who excels in this role will: Ensure compliance with standards of the accrediting bodies, state mandated revised code and the CMS Conditions of Participation. Promote interprofessional synchronization throughout the acute episode. Complete effective handoffs to the next level of care or community to ensure continuity of care. Proactively assess, identify and work to mitigate the risk posed by barriers to patients' transitions of care. Work with the patient and/or care partner(s) to develop a comprehensive, individualized transitional care plan. Participate and lead in patient rounding activities, huddles and committee meetings. Minimum qualifications for the ideal future caregiver include: Graduate from an accredited school of Professional Nursing (Diploma, ADN, BSN program) Current state licensure as a Registered Nurse (RN) Basic Life Support (BLS) Certification through the American Heart Association (AHA) or American Red Cross Three years of recent RN experience Preferred qualifications for the ideal future caregiver include: Bachelor's of Science in Nursing (BSN) CTM, ACM, CCM or AHA Certification by an approved entity Two years of recent experience in utilization review, quality or care management Knowledge of acute care and accreditation requirements Experience in discharge planning and quality improvement Physical Requirements: Requires normal or corrected hearing and vision to normal range. Ability to perform work in a stationary position for extended periods. Ability to communicate and exchange accurate information Ability to work with physical records, such as retrieving and filing. Ability to operate a computer and other office equipment. Ability to travel throughout the hospital system. Requires normal or corrected hearing and vision to normal range.
