About the role

The Change Realization Consultant is an individual contributor that is skilled in change leadership and partners with the Senior Change Realization Consultants and AVP of Change Realization in facilitating and sustaining change. This position is responsible for planning, implementing, and overseeing change initiatives with an organization. Job Duties Organize and support change initiatives such as education and change implementation planning session. Co-facilitate education programs Integrate change realization activities into project plans. Coordinate and support change function project plan activities Create deliverables for communications, roadmap, education and resistance management.. Create calendar of change events. Researches best practices and supports in the development of change leadership materials for organization-wide programs Data and Reporting: Track feedback and success metrics. Report on adoption, utilization, and proficiency. Track and maintain change initiatives, change leaders, and change agents Engagement: Models Change Agent behaviors and habits to instill change culture throughout the organization. Build strong relationships and communications to position process change work as a key enabler to strategy to execution efforts. Model Jefferson values and habits in change efforts

Requirements

  • Bachelor's Degree in Healthcare Admin, Education or related field
  • 2 years of experience leading and supporting teams through change initiatives or within an organization development field which may include learning and organization development
  • Familiarity with change internal or industry standard change management methodology like CAP, Prosci, SEF, etc. Change Management certification preferred
  • Master's Degree in Healthcare Admin, Education or related field
  • Strong communication and analytical skills
  • Active listening and relationship building skills
  • Experience with large scale change efforts
  • Ability to influence and lead change across diverse stakeholder groups
  • Keen attention to detail and advanced organizational and time management skills and ability to develop and manage a project plan incorporating change management principles
  • Designs evaluation methodology and tools to measure program effectiveness and provide needed reports
  • Possesses excellent project management and organizational abilities
  • Proficiency with Microsoft 365
  • ICGB - Certified Lean Six Sigma Green Belt - International Association for Six Sigma Certification
  • CCMP - Certified Change Management Professional - PROSCI
  • GE Master Change Agent - General Electric
  • Physical Demands
  • Lift and carry 25 lbs. frequent sitting/standing, frequent keyboard use, *patient care providers may be required to perform activities specific to their role including kneeling, bending, squatting and performing CPR.
