- Essential Responsibilities
- Demonstrates compliance with Code of Conduct and compliance policies, and takes action to resolve compliance questions or concerns and report suspected violations.
- Monitors heart failure program & performance data.
- Coordinates and evaluates effectiveness of the multi-disciplinary Performance Improvement for the heart failure Program, providing recommendations for improvement.
- Plans and establishes goals and objectives for the heart failure program, in conjunction with the Medical Director, Cardiac Leaders, and ED Leaders.
- Provides supervision and education to ensure the proper functioning of the heart failure Program; serves as a resource to the health care team members involved in caring for all cardiac related patient population.
- Monitors heart failure accreditation requirements and ensures timely progress. Collaborates with involved departments to fulfill accreditation.
- Participates in Eisenhower's heart failure & critical care committees; works closely with the Medical Director to assist in establishing the standards of quality for heart failure care.
- Demonstrates implementation skills in accordance with Eisenhower Health's Standards of Care, policies, procedures, and Nurse Practice Act.
- Maintains current knowledge of cardiac-related best practices and the latest clinical practice guidelines, including legislation and regulations that impact heart failure care.
- Coordinates and attends heart failure education presentations for those caring for heart failure patients internally - and for referring facilities as needs and opportunities are identified.
- Consistently documents in accordance with EMH policies and procedures.
- Assesses and analyzes outcome measures for continued heart failure accreditation.
- Demonstrates competency in heart failure patient flow.
- Provides accurate and precise data reporting of heart failure patient encounters to appropriate registries.
- Identifies educational needs and provides comprehensive education pertaining to heart failure intervention, prevention and rehabilitation for inpatients and community.
- Collaborates with nursing management, physicians and the Education Department to identify gaps in processes, develop and implement staff education related to heart failure issues.
- Coordination of heart failure patient care between the ED and other departments as needed.
- Coordinates research, collects and gathers data for reporting.
- Complies with departmental plan to meet budget goals.
- Demonstrates knowledge of Department of Nursing and Joint Commission required quality indicators, performance improvement and patient safety goals.
- Oversee heart failure data abstraction and reporting of metrics related to the heart failure process for the ongoing term of heart failure accreditation.
- Serves as liaison to administration and represents the heart failure program on various hospital and community committees to enhance and foster excellence in heart failure care.
