Job Title: Social Worker - Hospital Team Location: Slough Pay Rate: £36.81 per hour Job Description: Liquid Personnel is currently seeking a compassionate and skilled Social Worker to join the Hospital Team on behalf of our client in Slough . The successful candidate will support patients and families in navigating the healthcare system and accessing essential resources. This role involves close collaboration with medical staff to ensure holistic care during hospital stays and post-discharge. What will your responsibilities be? Conduct psychosocial assessments of patients to identify their needs and develop appropriate care plans. Provide emotional support and counseling to patients and their families, addressing concerns related to illness, hospitalization, and discharge planning. Collaborate with medical staff, including doctors, nurses, and therapists, to coordinate patient care and ensure comprehensive support. Assist patients and families in accessing community resources, financial assistance, and other support services. Facilitate discharge planning, including arranging for home care, rehabilitation, or other post-hospital services. Advocate for patients' rights and needs within the hospital setting and in the community. Maintain accurate and up-to-date records of patient interactions, assessments, and care plans. Participate in multidisciplinary team meetings, case conferences, and training sessions. Stay informed about current best practices, policies, and legislation related to social work and healthcare.
