Licensed Practical Nurse (LPN), Transitional Care - ElderONE (Part-Time, Days/Evenings)

About the role

The Licensed Practical Nurse is responsible for providing direct patient care, usually under the direction of a Registered Nurse, in accordance with established plans.

Responsibilities

• -Conducts and documents a thorough observation of each patient's medical status upon admission and throughout the patient's course of treatment.
• -Assists in the implementation of the patient's treatment plan under the direction of the RN.
• -Provides routine nursing services for patients as directed by the RN.
• -Complies with the evaluation, treatment, and documentation guidelines of the department.
• -Completes required documentation in an accurate and timely manner.
• -Provides regular patient status updates to the appropriate personnel.
• -Attends and participates in department/facility meetings as required.
• -Acts in compliance with hospital, regulatory, and professional standards and policies.
• -Maintains clinical competency by pursuing continuing education opportunities.
• -Performs other duties as assigned.
• REQUIRED LICENSURE / CERTIFICATION SKILLS
• Graduate from an accredited School of Practical Nursing.
• Current LPN licensure in State of New York.
• Basic Life Support (BLS) Certification from the American Heart Association (AHA).

Requirements

• 1 year experience
• 1 year of experience working with a frail or elderly population preferred.
• Strong communication, customer service, and interpersonal skills highly desired.

• PHYSICAL REQUIREMENTS:
• M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.

Job Title : Licensed Practical Nurse (LPN), ElderONE (Part Time, Days/Evenings) Department : ElderONE - Transitional Housing Location: ElderONE - St. Mary's Campus: 89 Genesee Street, Rochester, NY 14611 Hours per Week: 24 Schedule: Every other weekend (Saturday and Sunday) working either a 7:00 AM-2:00 PM or 5:00 PM-9:00 PM shift, with the remaining hours completed through a combination of 6:00 AM-8:00 AM and 5:00 PM-9:00 PM shifts during the week to meet the 24-hour schedule. Every other holiday required.
