About the role
The Outpatient Social Worker provides customized clinical interventions, that are evidence-based, to treat underlying, root causes of psychosocial and environmental symptoms. Serves as a liaison between medical professionals and patients to increase collaboration and positive medical outcomes.
Responsibilities

Requirements
- Master's Degree in Social Work
- Required Licensure/Certification:
- NYS LMSW or NYS LCSW
- REQUIRED QUALIFICATIONS:
- Required Degree: Master's Degree in Social Work
- Required Certification/Licensure: NYS LMSW or NYS LCSW
- One (1) to three (3) years of social work experience in a health care or home care setting
- Compassionate, warm and patient focused
- Exceptional documentation and planning skills
- Excellent communication and interpersonal skills
