Clinical Pharmacy Specialist, Advanced Heart Failure and Heart Transplant, Sign-On Bonus Eligible Opportunity for the position to be a Senior Clinical Pharmacist Specialist, dependent upon qualifications and experience of the candidate. Job Overview This pharmacist is a member of the Department of Pharmacy with a primary focus of providing comprehensive pharmacy services to patients admitted to Tufts Medical Center with Advanced Heart Failure , including patients before, during, and following heart transplantation and left ventricular assist device (LVAD) implantation. This pharmacist will work with other clinical pharmacists, including working in tandem with an additional inpatient Advanced Heart Failure pharmacist, and with the broader multidisciplinary teams to ensure optimal patient care. The pharmacist in this role will provide leadership for implementing and maintaining drug policy guidelines and protocols. This pharmacist will be responsible for maintaining a clinical practice while educating Tufts Medical Center pharmacists, pharmacy residents, pharmacy students, and other care team members. Responsibilities Include Working with members of the pharmacy, medical and surgical staff to care for advanced heart failure patients including heart transplant recipients and LVAD patients. Working with inpatient pharmacists to ensure optimal operational and clinical pharmacy services to every patient for whom they have responsibility, including review and approval of medication orders Participating in multidisciplinary rounds with the Advanced Heart Failure Team(s) Completing pre-transplant evaluations for prospective heart transplant and LVAD recipients. Evaluating and documenting post-operative medication reconciliation and the drug therapy plan for heart transplant recipients. Participating in and documenting discharge medication reconciliation and patient education. Completing index heart transplantation medication discharge teaching and index LVAD implantation warfarin/enoxaparin teaching in compliance with CMS regulations Facilitating transitions of care between cardiology services (i.e., inpatient and outpatient). Working with members of the heart failure and transplant team to facilitate patient access to medications. Providing support, in cooperation with other clinical pharmacists, to outpatient cardiology clinics. Maintaining selected institutional guidelines, protocols and policies. Ensuring compliance with quality and regulatory standards. Participating in weekly Heart Transplant Listing and LVAD meetings, monthly Quality and Performance Initiative (QAPI) meetings, and morbidity and mortality conferences. Serving as a preceptor for PGY1 and PGY2 residents and pharmacy students. Job Description
