Community Occupational Therapist - Bristol

Responsibilities

• Support discharge planning , prevent admissions, and manage frailty cases.
• Provide interventions for patients with falls risk, neuro, and orthopaedic needs .
• Work collaboratively with the multidisciplinary team to deliver patient-centered care.
• Promote independence and functional recovery through evidence-based practice.

Requirements

• HCPC registration as an Occupational Therapist.
• Proven experience in acute and/or community settings.
• Valid driving licence and access to a car (essential for community work).
• Strong understanding of discharge pathways and admission avoidance.
• Excellent communication and teamwork skills.
