Job Title: Social Worker - Adult Community Team Location : Wakefield Rate : £35 per hour The Wakefield Council is seeking a qualified social worker with experience of working with adults to join their team. What will your responsibilities be? In this role, you will engage in strengths-based conversations with service users, families, and advocates to determine eligibility and support planning. You will undertake Capacity and Best Interest Assessments in line with the Mental Capacity Act 2005, conduct NHS Continuing Healthcare checklists, and participate in Decision Support Tool (DST) meetings. Additionally, you will manage a caseload effectively within a fast-paced environment and collaborate with a multi-disciplinary team, including health, pharmacy, housing, Age UK, and Carers Wakefield.
