Liquid Personnel is recruiting a Social Worker for the Preparing for Adulthood Team for its client in Bury. What will your responsibilities be? Supporting transitions planning - working with young people (14-25) to develop person‑centred transition plans that promote independence, wellbeing and long‑term outcomes. Completing Care Act assessments - assessing eligible needs as young people move into adulthood, creating strengths‑based support plans and coordinating appropriate adult services. Multi‑agency coordination - collaborating with education, health, SEND teams and families to ensure smooth transitions and consistent support across children's and adults' services.
