Liquid Personnel is seeking an experienced and dedicated Adults Social Worker to join our client's Countywide Reviewing Team, based in East Riding of Yorkshire. What will your responsibilities be? In this role, you will: Conduct proactive reviews of care arrangements: Assess residents in care homes with 1:1 support and individuals receiving Direct Payments to ensure packages are appropriate, least restrictive, and proportionate. Update assessments and care/support plans: Visit individuals in their home environment, review their needs, and revise documentation to reflect current circumstances. Submit funding requests and liaise with providers: Work collaboratively with care providers to adjust support levels, reduce unnecessary restrictions, and ensure funding applications are accurate and timely.
